Some of our professionals choose to post their intake paperwork online to save time during the initial appointment.  Please check with your professional to see what paperwork he/she would like you to print and complete prior to the first appointment.

 Dr. Lorrie Beever's Paperwork

-The "Adult Intake Sheets- Part 1 and Part 2" should be filled out by adult clients and returned at the first visit.

-The "Child & Adolescent Intake Forms- Part 1 and 2" should be completed by parent/guardian and returned at the first visit if Dr. Beevers is treating a child/adolescent.

-The "Authorization for Use or Disclosure of Information" form should be completed and returned at the first visit if Dr. Beevers is filling insurance for you or providing information to an outside source.

-The "Psychotherapy Information Sheet" should be completed and returned by all clients at the first visit.

Dr. William MacGillivray's Intake Paperwork

-If possible, please complete the requested paperwork and bring it with you to the first session.  If you do not have access to a printer, Dr. MacGillivray will provide you with copies.
